What is a Scar?

The formation of a scar starts with trauma to the skin which damages the dermis and causes bleeding.  The skin is made up of three layers.  The deepest layer is called the dermis.  When the dermis is injured by a deep wound, a scar naturally occurs to bind the edges of the wound together.  All deep incisions through the dermis will form a scar.

Scars are the physiological healing result of any trauma to the skin where the dermis (deep layer) of the skin has been injured. During the healing process special healing cells called Fibroblasts migrate to the site of injury.

These Fibroblasts cover the damaged area and form collagen which is the building block of scar tissue. Normally, collagen fibers line up in a parallel fashion like spaghetti in a package, forming a bridge across the wound. This collagen material is constantly remodeling for almost two years. Sometime in the first two years, this bridge of collagen tissue will gradually contract and pull the edges of the wound tightly together. This process results in a scar that is flat and very thin. The scar then becomes dormant.

An abnormal scar occurs when too much scar tissue is produced or the collagen fibers lay down in disarray much like cooked spaghetti. These hypertrophic (and keloid) scars are reddish and raised. They can itch and cause pain in the worst cases.

Vitamin C is essential for collagen biosynthesis. Practical Scar Treatment Tips

Avoid putting strain on a wound site during healing: Putting strain on a wound site can slow and hinder the healing process and lead to more prominent scarring. Stretching the skin may open/ widen the wound and increase healing time.

When applying scar gel, a good rule of thumb is to spend 3-5 minutes massaging SCARX into the skin during each application. We suggest using both circular (clock wise and counter clock wise) motions as well as up and down and sideways motions. This will help break down scar tissue.

Abstract

Topical self drying silicone gel is a relatively recent treatment modality promoted as an alternative to topical silicone gel sheeting. Thirty patients with scars of different types including superficial scars, hypertrophic scars, and keloids were treated with silicon gel application. The results of the self-drying silicone gel have been satisfactory.

Abstract

To assess the efficacy of topical silicone gel in the management of scars, we conducted this meta‐analysis. The systematic search was performed on PubMed, Web of Science and Embase, and six randomised controlled trials with a total of 375 patients were involved. The outcome data of Vancouver Scar Scale were extracted from the studies and their effect sizes were calculated using Review Manager 5.3. As a result, topical silicone gel significantly reduced pigmentation, height, and pliability scores postoperatively compared with placebos or no treatment (Pigmentation: standard mean difference [SMD] = −0.55 [−0.83 to –0.26], P = .0002; Height: SMD = −0.73 [−1.02 to –0.44], P < .00001; Pliability: SMD = −0.49 [−0.95 to –0.03], P = .04). Topical silicone gel and silicone gel sheet were comparably effective (P > .05). The performance of topical silicone gel and other non‐silicone topical treatment was also similar (P > .05). In summary, topical silicone gel was effective in post‐operative scar prevention.
